Investigation Summary
On January 4, 2008, Employee #1 was walking from the cafeteria. The weather was severe with wind and rain. Employee #1 was carrying a cup of soup in one hand and an umbrella in the other hand. Employee #1's left foot slipped on leaves by the fountain area. She fell on the pavement on her right shoulder. Employee #1's arm was fractured in four places. She was taken by ambulance to San Jose Regional Hospital. Employee #1 underwent surgery on her arm the following day.
