Investigation Summary
On April 28, 2008, Employee #1 was standing next to a forklift that was lifting a dumpster. The dumpster was overfilled with recyclable materials, and there was a dishwasher on top of the materials. Employee #1 was injured when the dishwasher fell and struck him on the head. He was hospitalized until his death several days later.
