Investigation Summary
At approximately 5:00 a.m. on September 19, 2008, Employee #1 was walking from the rear of a band saw to the side of it. He tripped and his arm entered the area between the frame of the saw and the shuttle mechanism, as it was traveling forward to the point of operation. The employee experienced soft tissue and nerve damage in his right arm, as a result of the accident. Employee #1 was hospitalized.
