Employee Is Caught Between Forklifts, Crushes Pelvis

At approximately 5:30 a.m. on August 11, 2008, Employee #1 was in the ticket receiving aisle in the distribution center, facing south. Employee #1 was off of his powered industrial truck (PIT) (Crown Model Number 60FCTT-180) filling out load-receiving papers on the right rear side of the PIT. A coworker had just received his order ticket from the receiving station, facing north on the west side of the aisle. The total width of the aisle was approximately 16 feet in the area. The coworker was backing PIT Number 2 (Crown Model Number 60FCTT- 198) from the west to the east side of the aisle (turning radius of 80.9 in.), in order to travel south back down the aisle (the width of both PITs was 40 in.). The coworker failed to yield to Employee #1 and his PIT struck Employee #1, creating a caught-between hazard between the two PITs. Employee #1 received crushing injuries to the pelvis area and was life-flighted from the scene. Employee #1 was hospitalized.
