Employee Is Crushed in Press Shuttle

On June 20, 2008, an employee was making an adjustment to a clip loading device. He failed to lockout and tag out a hydraulic press. After entering the interlocked area to conduct trouble-shooting, the employee attempted to make the necessary adjustment without isolating the press and or shuttle. While the employee was on a step ladder making the adjustment, the shuttle of the press came forward and crushed the employee between the shuttle and a roller bar, causing rib fractures, a deflated lung, and contusions. A coworker was in the area, and allowed for the shuttle to retract back to the home position to allow the employee to be released from the crushing incident. The employee was hospitalized and treated for his injuries.
