Investigation Summary
A 58-year-old women was operating a Toyota stand-up narrow-aisle reach truck (Model No. 7BRU18, Serial No. 32388). After placing a full pallet on a shelf of a rack system, the employee operated the reach truck in reverse and backed into a storage rack. The chassis of the truck went under the shelf and the overhead guard went above the shelf. The employee was caught between the reach truck and a pallet of material. The operator suffered severe neck injury, and the employee died from the injury. No witnesses were present at the time of the incident. No environmental conditions contributed to the incident.
