Employee Is Paralyzed When Machinery Unexpectedly Starts Up

At approximately 10:30 a.m. on February 19, 2008, an employee was working for a firm that manufactured automotive carpeting. The carpet being produced by one cell was misaligned when it was being sent on at a transfer point. The employee waited at the carpet unloading area until the machine's cycle was complete. When the cycle was complete, the carpet unloading zone's presence-sensing devices were muted. With the presence-sensing devices disabled, the employee entered the caged area where robots and other moving machinery were positioned. The employee stood on the rotating carousel, stationary at the time, to observe and inspect the suspected cause of the carpet's misalignment. Because the presence sensing devices did not detect anyone's presence, the machinery and carousel activated. When the carousel started to rotate, the employee fell onto the carousel, sustaining the permanent paralysis of his lower extremities.
