Investigation Summary

Investigation Nr: 202088480
Event: 01/08/2008
Employee's Finger Is Injured by Trimmer Machine

On approximately January 8, 2008, Employee #1, a 29-year-old, English-speaking worker who was employed full time by a plastic bottle manufacturer, had been assigned the task of monitoring a Graham bottle trimming machine. After a bottle was molded, it went to the trimmer section of the machine, which removed excess flash from the bottle. Employee #1 had noticed some flashing hung up in the machine and feared that the piece could cause defects in other bottles. Employee #1 reached through an approximate three-inch opening to get the piece, and his left, third finger was crushed by the trimmer's rotating parts. His finger was apparently bruised and/or abraded. The three-inch opening in the guard was apparently placed there to facilitate adjustment of the machine. Apparently, no other workers were present at the time of the accident. The work area was well illuminated and had dry floors.
