Employee Is Killed When Caught in Agitator

At approximately 2:30 p.m. on February 18, 2009, an employee at a pig breeding and gestation facility was caught up in the three sets of blades of a feed mixing tank while performing routine power washing of the tank. The employee had entered the top hatch of the tank with high pressure water cleaning wand and line. The wand was found at the bottom of the tank with the employee and the line wrapped around the mixing blade shaft. The employee was killed.
