Employee Is Killed in Fall from Secured Deck

On June 21, 2008, an employee was called over to tack weld several short sections of steel deck that had been cut and fitted around a stairway opening. When the employee approached, he stepped on one section of the unsecured deck. When alerted to the fact by a coworker, the employee responded by reaching down to grab and connect to a horizontal lifeline. The action dislodged the piece of decking and the employee fell. In an attempt to catch himself, the employee twisted around and grabbed a beam as he fell. As he caught the beam, his momentum caused him to swing out over the stairway opening and subsequently fall into the opening. The deck immediately below the point where the employee fell was 12 feet. The stairway opening at the level from where the employee fell and at the deck immediately below were both fully guarded. The employee fell 30 ft to his death.
