Investigation Summary
A forklift operator was driving a Kalmar forklift truck into position to unload pipe onto a pipe rack. Employee #1 walked up to the front of the forklift and his foot was run over. He fell to the ground and the back left wheel of the forklift ran over him. He was killed.
