Investigation Summary
At 3:00 p.m. on January 2, 2008, Employee #1 descaled corrugated sheet pile using the Pangborn machine and his Supervisor operated the forklift transporting the cleaned plate from the shop (the Pangborn machine is an indoor blasting machine that measures 16'4" long x 12' wide x 13'7" tall, similar to a sand blaster that uses small steel metal shot to clean and descale metal plate prior to painting; the metal plate is lifted by an overhead crane, turned length wise and placed into the blasting machine.) During operation, the Pangborn machine tripped the breakers and Employee #1 climbed on top of the machine to troubleshoot the cause. Employee #1 asked the Supervisor for a length of pipe to use as his wrench's cheater pipe to make an adjustment on the machine. The Supervisor found a length of pipe and turned around to see Employee #1 fall 14 feet, head first, onto the concrete below. Employee #1 suffered massive head damage; he was removed from life support and died on January 3, 2008.
