Employee Gets Crushed by Forklift

On January 24, 2008 Employee #1 of Kimura got struck by a forklift. Employee #1 drove his cushion-tired fork truck (per manual not approved for this terrain) into the yard to retrieve a jig. The forklift bogged down into the dirt/gravel and became stuck as the ground was soft from rains earlier in the week. Employee #1 could not free the lift by himself and went back into the plant seeking help from Employee #2. Employee #1 grabbed a chain and Employee #2 followed behind driving a Yale forklift to help pull the Toyota forklift free. They attempted to pull the Toyota forklift free by connecting the chain to the drawbars on the rear of each lift (per operator's manual instructions). These efforts were unsuccessful and they then transferred the chain from the rear drawbar of the Toyota to the front mast down low near the forks. Employee #2, after being signaled, tried to pull the lift forward to free it. As the chain went taut Employee #2 looked back to see if it was pulled free and saw Employee #1 jumping from the Toyota forklift as it overturned. It fell on top of Employee #1 crushing him against the ground. He died at the scene.
