Investigation Summary
At approximately 8:40 a.m. on October 19, 2008, Employees #1, #2, and #3 entered the substation of a rail yard to reset an oil-based switch (breaker/2400 volts) that had tripped. Two of the employees were using a steel spud bar to manually reset the oil-based switch, when there was an arc flash that ignited the (mineral) oil inside the switch, resulting in a fire ball. All three employees suffered severe burns and had to be airlifted to burn units. Employee #1 was killed, and Employees #2 and #3 were hospitalized. The employees were not wearing any electrical personal protective equipment.
