Employee Is Killed When Pinned by Fork Truck

On December 5, 2007, Employee #1 exited his fork truck leaving it running and in reverse without setting the parking brake. The fork truck automatically went to neutral when the driver was not in the seat. The fork truck was on a slight incline which allowed it to roll forward due to gravity and the weight of the lift. He was struck by and caught between the mast of the fork truck and a stack of wooden pallets. Employee #1 was killed.
