Investigation Summary
On November 8, 2011, Employee #1 was standing on a work platform that was being lifted by a Lull telescopic all-terrain forklift. The platform was at a height of approximately 30 ft when the forklift tipped over and Employee #1 fell to the ground. He died of injuries sustained in the fall.
