Investigation Summary
At approximately 12:15 a.m. on April 21, 2008, Employee #1, a safety man, was operating a Caterpillar 988F wheel loader to move hot slag material from the knocking station to a cooling pile. As the employee dumped the load, some of the hot material rolled into water which had accumulated next to the cooling pile. The resulting explosion blew the widows out of the cab and set the loader on fire. The employee sustained third degree burns to his right arm and first and second degree burns to his neck and lower abdomen. Employee #1 was hospitalized. Employee was reportedly wearing flame resistant pants, but not a flame resistant shirt.
