Employee Is Killed When Crushed by Steel Plate

At approximately 7:00 a.m. on February 3, 2009, Employee #1 was welding 1-in. thick steel stiffeners onto a large (19 ft 11 in. X 7 ft 11 in.) 2-in. thick steel plate. The steel plate was standing on its end and was tack-welded to two large steel horses, which were meant to keep the plate upright and stable. While Employee #1 was welding the stiffeners into place, the tack welds broke loose, and the steel plate fell onto Employee #1, instantly crushing him to death.
