Employee Dies from Asphyxiation

On February 26, 2008, Employee #1 and a coworker were performing maintenance work. After trying to clear a blocked intake, they decided to go to lunch and come back and clear the blockage. The coworker went to start the project back up, and Employee #1 started to walk back down the intake wall where he had been trying to attach a line to the tree which was blocking the intake. When the coworker returned from the plant Employee #1 was nowhere around and emergency services were called. He was found three days later dead due to asphyxiation.
