Investigation Summary
On February 29, 2008, Employee #1 of Pyco Industries, Inc., fell into a four foot deep pit containing boiling water and, or oil and several caustic, acid based chemicals. Employee #1 extricated himself from the pit and called 911. Employee #1 was transported to a local burn unit with scalding and chemical burns over of his lower body. Employee #1 succumbed to his injuries on March 4, 2008.
