Investigation Summary
On June 23, 2008, Employee #1 was riding in the cargo box of a John Deere Gator utility vehicle with two coworkers. The vehicle had accommodations for two persons only. They were on a descent on a gravel road. When they arrived at the concrete turn pad, Employee #1 was thrown from the vehicle, striking his head on the concrete pad. He was hospitalized and died three days later.
