Investigation Summary
On November 6, 2007, an employee and two coworkers were working on top of a platform that was being held up in an elevated position while they were repairing a Burr House door. The platform was not secured to the forklift. As the employees moved on the platform, the platform became unbalanced as the center of gravity changed, The platform fell toward the Burr House wall, the end of the platform pinned the employee's head against the wall. The employee was killed.
