Investigation Summary
On August 2, 2007, Employee #1 was laying corrugated sheet metal on a flat roof. A piece he just place was not straight and he kicked it with the toe of his boot to realign it. This cause the piece of sheet metal to slip away from the adjoining piece. Employee #1 lost his balance and fell through the gap he had created by kicking the sheet metal. He died of injuries sustained in the fall.
