Employee Is Killed in Fall When Guardrail Breaks

On June 27, 2007, Employee #1 and a coworker, maintenance workers, were assigned to repair a welded pipe guardrail on a mezzanine approximately 17 ft above a lower level. Force was applied to the guardrail in an effort to straighten it and the welds at the guardrail uprights broke. The guardrail released from its connecting brackets and Employee #1 fell to the lower level and died.
