Employee Is Crushed While Repairing Machine

On September 11, 2007, an employee was found, unresponsive, under the belt on a thermo forming machine. The employee, a maintenance technician, was repairing a cutter on the machine. The machine was not locked out. The back part of the machine, the roll feed unit, was powered down, the front part of the machine, the oven, where the belt was located was powered up. Other coworkers were cleaning the belt on the front of the machine. After cleaning the belt, a coworker moved the belt back to the home position. It appeared that the employee had climbed into the machine to repair the cutter and was crushed between an idle roller and the belt as it was moved to the home position. The employee sustained injuries to his abdomen. The employee died on September 13, at approximately 12:40 a.m.
