Employee killed when crushed by falling scaffold platform

At approximately 12:45 p.m. on September 12, 2007, Employee #1 was preparing to caulk a window that was being installed. He stepped onto a scaffold supported by a rough terrain forklift when the scaffold tipped to one side, overturned, and came off the forks. Employee #1 fell approximately 10 ft 3 in. to the ground, and the scaffold platform landed on top of him. He was killed instantly. Employee #1 was not tied off to the scaffold, and the scaffold platform was not secured to the forks of the lift truck.
