Employee Crushed and Killed by Falling Boom

The employee was working on a truck that picks up recycling dumpsters. He had lifted the cab and the boom via hydraulics to inspect for an air leak. He climbed under the boom without setting the boom support arm lock in place and bumped the switch to lower the boom with his knee. The boom came down on him, crushing and killing him.
