Investigation Summary

Investigation Nr: 200452290
Event: 08/29/2007
Employee Is Struck and Killed by Tractor Trailer

On August 29, 2007, Employee #1, a mechanic, was asked to place a spare tire in the rack of a tractor trailer. He also removed ten landscape timbers from the bed of the trailer before the driver left. The driver walked from the lounge to the cab of the truck, which was locked and parked at the wash station, and shouted goodbye to the mechanic. He stated that the mechanic was off to the side, stacking the landscape timbers. The driver started the truck and drove off. At some point, it appears that the mechanic had come back towards the trailer, as the trailer struck and crushed Employee #1 at the wash station. Another mechanic found him dead approximately thirty minutes later.
