Investigation Summary
On November 8, 2007, Employee #1 was replacing bulbs in malfunctioning street lamps. The employee had energized the lines (by covering the photocell) to determine which lamps were malfunctioning, but he did not deenergize them prior to initiating the work. He came in contact with a 480 volt overhead power line, and he was electrocuted. Employee #1 was killed.
