Employee killed when struck on head by falling beam

On August 2, 2007, Employee #1 and a coworker were setting a 6 in. by 9 in. by 8 ft long beam on top of posts of the same size. The beam was attached to one post using a metal connector bracket and then pinned with a nail. The other end of the beam was sitting in a bracket on top of the adjacent post. Employee #1 was standing under the beam when he asked his coworker to push it toward the house so he could set a rafter on top of it. As the coworker pushed on the end of the beam, it slid off the top of the post and swung down, striking Employee #1 on the head. He died of injuries sustained in the accident.
