Employee Is Killed When Pinned by Forklift Mast and Guard

At approximately 12:55 p.m. on April 17, 2007, Employee #1 was using a forklift to load a pallet into a beverage delivery truck. The pallet contained boxes of bottled beer. He was having difficulty trying to place the pallet into the side compartment of the beverage delivery truck and had broken the tension rod for the roll down compartment door. The employee stopped the forklift. With the motor running, he stood up on the dashboard to look at the top of the boxes and the top of the compartment door. As he was leaning forward, his leg moved backwards and struck the lever that controls the tilt of the mast. As the mast moved rearward, the employee was pinned by the top of the mast and the overhead guard of the cab. Employee #1 was killed.
