Employee Fractures Head When Struck by Flying Object

On March 30, 2010, Employee #1 was completing the repair of the 6-in. control valve and actuator assembly. Employee #1 was connecting the valve section of the assembly to the actuator. Employee #1 was applying air pressure to the top side of the actuator diaphragm to lower the actuator stem so that it could be reattached to the valve stem at the proper setting (setting the travel). This involved adding pressure, adjusting the stroke and then the pressure is released. The actuator pushes down on the diaphragm, which pushes down on the spring in the lower diaphragm and this action creates stored energy. During this part of the repair the actuator exploded causing the top of the actuator and spring inside of the actuator to fly into the air. Employee #1 was struck in the head and was seriously injured by a part of the actuator. Employee #1 sustained a fracture and was hospitalized.
