Employee Fractures Arm in Fall

On January 14, 2010, Employee #1 was located in a cafeteria at her employer's facility for her morning break. As Employee #1 prepared a snack herself, she fell and injured her arm. Employee # 1 was taken to Kaiser in Richmond, California, where she was hospitalized and treated for her injury. Approximately five days later, Employee #1 was admitted to Valley Care Hospital in Pleasanton, California, where she underwent surgery on the injured arm.
