Investigation Summary
At approximately 9:10 a.m. on December 4, 2008, an employee, a heavy equipment mechanic, was in the field repairing a left air brake chamber for a paving truck. The employee was using a compressed air hose to install the parking brake spring caging tool into the brake chamber. The compressed air hose nozzle lost contact with the chamber causing the compressed air holding the spring to release. As a result, the lock tool nut and washer moved rapidly and the employee sustained a 0.75 inch amputation of the tip of his left index finger.
