Employee Amputates Two Fingertips in Machine

At approximately 6:00 p.m. on September 13, 2008, Employee #1 was running the East end of the Gift Box Line, Box Erector Number 1T80. This line was set up to assemble gift boxes, approximately one 1-in. deep by 6-in. wide by 14.25-in. long. During operations the Box Bottom Former, had jammed three times and maintenance was called to fix the jamming problem. Upon fixing the jammed Box Bottom Former maintenance personnel left the area. At that point, Employee #1 experienced another jam at the Box Bottom Former and reached under the interlock-protected door, in order to clear the jammed Box Bottom Former. After the jam was cleared, the machine reactivated and amputated the tips of the third and fourth fingers on Employee #1's left hand. Employee #1 was hospitalized.
