Employee Is Killed in Fall from I-Beam

On October 10, 2008, an employee was working 40 feet above ground connecting steel I-beams. Coworker #1 was a member of his crew. The coworker was also working 40 feet above ground on the opposite side of the structure. The employee and the coworker were sitting on I-beams preparing to connect an I-beam that was hoisted up to them by Coworker #2. Neither the employee nor Coworker #1, attached their harnesses or lanyard to an anchor as there was no anchor point there. The employee fell from the I-beam that he was sitting on. Neither Coworker #1 nor Coworker #2 witnessed him fall. Coworker #1 said he was busy working on his end of the beam, when he heard the employee hit the ground. Coworker #2 said that he caught sight of him falling out of the corner of his eye. There were eight workers working at the sight; no one witnessed what caused the employee to fall. The employee did not have his lanyard secured, which would have prevented his fall. The employee was killed.
