Investigation Summary
On March 5, 2008, Employee #1 was using a forklift to load a storage rack into a large drive-in freezer, adjacent to the warehouse area. It is believed that he attempted to climb up on the articulating section of the fork assembly to remove a box that had fallen into this section. He fell off of the forklift and sustained a concussion. He was hospitalized for his injury. There were no witnesses and Employee #1 declined to make a statement.
