Employee Fractures Leg When Shoe Is Caught in Rubber Mat

On November 13, 2008, Employee #1 was making copies, when she caught the heel of her shoe in a hole in a rubber mat in front of the copier. The employee fell and fractured her right leg. She was hospitalized with her injury. Further investigation revealed that the rubber mat was not well maintained and had a 2-inch by 0.5-inch hole in it.
