Investigation Summary

At approximately 7:30 a.m. on August 14, 2008, Employee #1, an apprentice carpenter, was working on jobsite where a new pump station was being constructed. Employee #1 was standing on the platform of a bracket scaffold that was hung onto the walers of the west form panel for a concrete wall. The platform was no more than 7-ft height and was not equipped with guard rails. Employee #1 was standing on the platform installing walers. As he used a hammer to pull out a nail, he fell backwards off the platform unto a muddy ground. The emergency services were called. The San Francisco Fire Department responded to the jobsite. He was taken to Kaiser Hospital in San Francisco, California and was later transferred to University of California of San Francisco Medical Center to have surgery performed on his back bone. Employee #1 was hospitalized. Fall protection was not used and was not required at the height of the platform. No citations were given.
