Investigation Summary
At approximately 3:00 p.m. on June 26, 2008, Employee #1 was working at a sheriff's department as a deputy sheriff. He was on the 5th floor walking down a hallway. He walked in between two orange cones and took a step or two past the orange cones, where the floor was wet, slipped and fell. He was unconscious for a short period of time and was hospitalized with head injuries. He was discharged the following day and sent home. On June 30, 2008, he was admitted into another hospital and was released on July 3, 2008.
