Employee Is Injured in Fall from Secured Ladder

At approximately 7:55 a.m. on June 26, 2008, Employee #1, a roofer, was climbing a 28 ft aluminum extension ladder with an eight rung over lap. The ladder was triple tied at the top of the building. He was not carrying anything in his hands as he climbed. Approximately eleven rungs up the ladder, he could see the roof top and extended his right hand to grab the next rung. His left foot slipped, and he lost balance and tried to grab the ladder. He fell to the right, striking the right side of his head on the concrete. His hard hat was found a couple of feet away. He sustained a cut at the right eyebrow, four fractures to his head and two fractures to his back. Employee #1 was hospitalized.
