Employee's Arm Is Caught Between Chain and Sprocket

At 8:00 a.m. on October 6, 2007, Employee #1, a bakery employee, reached her hand into a DBE Food Equipment Inc. overhead proofer to remove bread dough that had doubled-up in the single dough baskets. Employee #1 caught her arm between the proofer's unguarded chain and sprocket. Employee #1's arm was lacerated, and she was hospitalized.
