Investigation Summary
Employees #1-#5 were attempting to unplug a plastic extruder. When Employees #1-#5 opened the hatch door of the FCM (Farrel Continuous Mixer), the ambient air in the area was introduced to the heated oil based carbon black within the extruder, resulting in a flash fire. Employees #1-#5 sustained various degrees of burns to their hands, face, chest, and torsos. Employees #1-#4 were sent to the Joseph M. Still Burn Center, while Employee #5 was not hospitalized.
