Investigation Summary
Employee #1 and other workers were discussing where they were going for lunch. Employee #2 and Employee #3 were in the cab and Employee #1 was on the passenger side running board. Employee #2 had Employee #3 look to the left and saw nothing, they looked back to the right and Employee #1 was gone. They assumed he had gotten in the back of the truck and as they began to pull into the road they felt a bump. They stopped and Employee #3 exited the truck and found the right front tire on top of Employee #1.
