Employee Heart Attack Causes Forklift Accident

On August 20, 2007, Employee #1 was operating a forklift that ran out of fuel. Employee #1 drove a second forklift to retrieve a full propane fuel tank. After placing the full propane tank on the forklifts forks, Employee #1 put the forklift in reverse and then suffered a heart attack while traveling in the reverse. The forklift Employee #1 was driving struck a parked forklift, ejecting him. Employee #1 struck his head on a pole. The County Coroner ruled the cause of death as a heart attack.
