Investigation Summary
On March 1, 2008, Employee #1, of Welders Unlimited, was removing an awning. He was on the roof while removing the awning during disassembly operations. He stepped through the insulation and fell approximately 18 feet to the concrete floor. Employee #1 died from the fall.
