Employee Killed When Struck by Sheet of Plywood

On June 25, 2007, Employee #1 was struck in the head by a sheet of 0.75 inch, 4 foot by 8 foot plywood and fatally injured. A bundle of plastic wood was dislodged by a trash receptacle that hit it. The plastic wood caused a domino effect, knocking over two other stacks of plastic wood, before knocking the plywood sheet into Employee #1.
