Investigation Summary

At approximately 3:20 p.m. on July 13, 2008, Employee #1, a forklift operator, was operating a Blue Giant PWB 45 pallet jack. He became lodged between the control arm of the machine and a loaded pallet, causing the control arm to squeeze his abdomen. He continued working until 11:33 p.m. that night. He worked approximately 25 hours over the next two days before seeing a doctor for treatment. The employer was notified on July 17, 2008 of Employee #1's injuries. Employee #1 was hospitalized for bruises and contusions to his abdomen.
