Employee Crushes Foot When Struck by Forklift

At 11:30 a.m. on October 29, 2007, Employee #1, a baler for a cardboard box manufacturing facility, was walking parallel to the corrugating machine, inside of the designated pedestrian zone. He was struck from behind by the left clamp of a Toyota Clamp Truck, Serial Number 60968, being driven by Employee #2. Employee #1 was airlifted to UMC in Fresno. The initial diagnosis of his injury was a degloving of the left foot. Employee #1 later developed complications, due to a preexisting condition, that resulted in amputation of his leg above the knee.
