Investigation Summary
On December 27, 2007, Employee #1, a general laborer at a motor coach outfitting company, was pushing oak material (4.5 in. W X 2.5 in. D X 28 in. L) through a blade from the in-feed side of rotating variable-angle dado blade (commonly called "wobble saw") mounted on a 8-in. table. The employee was feeding the material with his right hand, while pushing down on the material with his left hand to keep it from rising over the blade. A plywood table extension at the out-feed end of the table is 0.75 inch above the table that requires the operator to lift the material to clear it. At the time Employee #1 was lifting the material, a kickback occurred resulting in his left hand contacting the blade. The blade lacerated digits 1 through 4, and Employee #1 suffered bone loss and muscle and tendon damage as a result. Employee #1 had some experience in operating a table saw, although none using a dado. The employee was not provided training or supervision to perform this operation. At the time of the injury, the employer did not have worker's compensation insurance or an Injury and Illness Prevention Program. Root cause for this injury event was identified as the use of a prohibited wobble saw on an unsafe machine by an untrained operator. Legal counsel for Employee #1 notified the state of the previously-unreported injury on January 4, 2008.
